What is Selform MIS?
Selform MIS (Management Information System) is an online platform managed by TAMISEMI (Tanzania’s Ministry of Regional Administration and Local Government).
It is a digital system designed and developed by Regional Administration and Local Government (TAMISEMI) for simplifying selection processes to Form Four Students required to Join Form Five and Colleges in Tanzania.

Steps to Change Combination Form Five 2025
To change your Form Five subject combination for the 2025/2026 academic year using the TAMISEMI Selform system, please follow these steps:-
1. Access the Selform System: Visit the official Selform portal: https://selform.tamisemi.go.tz
2. Log In to Your Account: Enter your credentials to log in. If you haven’t registered yet, you’ll need to create an account by following the registration instructions provided on the portal.
3. Update Personal Information: Once logged in, ensure your personal details such as home address, mobile number, and email address are up to date.
4. Change Subject Combination: Navigate to the section for selecting subject combinations. Choose your desired combination from the available options. The system will display combinations based on your academic performance and eligibility.

Important Considerations
Eligibility Criteria
To switch to a new combination, you must have achieved at least a grade C in the relevant subjects. For instance, to opt for the PCB (Physics, Chemistry, Biology) combination, you should have secured a minimum of grade C in each of these subjects.

Be mindful of the application deadlines set by TAMISEMI. Ensure you make any changes within the specified timeframe.

NB: The Change of Combination depends on one key factor which is academic performance scored by students. The Performance directly affect your combination choice. For example, History B, Kiswahili A , Geography C and Chemistry D: YOU HAVE TO CHOSE HGK Not CBG basing on Nature of Your Performance.
