PROCEDURE FOR MAKING A REQUEST FOR THE CORRECTION OF NAMES
1.0 INTRODUCTION
The Tanzania Examinations Council processes applications for amendment of candidates’ names that were written incorrectly in the examinations of Primary Schools, Secondary Schools and Teacher Training Colleges.
The following is the procedure used in submitting applications and making amendments.
2.0 TERMS OF NAME AMENDMENTS
(a) Amendment applications should be submitted within a period not exceeding one year after test results released.
(b) The name to be amended must be different from the used name in the qualification test that enabled him to be registered.
So the purpose of the amendment is to do the name should be the same as the name used in the qualification test.
(c) Amendments aimed at adding a new name or removing/changing a name are not allowed.
(d) The only basis for correction of names is the name of the candidate written in the examination of reputation and not the name present in court oaths, birth certificates, certificates of marriage, baptism etc.
3.0 STEPS TO FOLLOW WHEN APPLYING
(a) Requests handled by the school, the Principal/Head Teacher should fill out the form correction or ‘Query Circular Form’ specifying the mistake made in the name and title the signature should be written.
(b) Applications handled by the parent/candidate; fill out the name correction form available on the website of the Examinations Council.
(c) Pay the name correction fee of Tsh.35,000/= (for one certificate) through banks NMB, CRDB and NBC or mobile networks (M-Pesa and Tigo Pesa) using ‘Control number’ to be issued by NECTA.
(d) Submit the NECTA application form attaching the wrong certificate/result document name and payment receipt.
4.0 RESPONSE TO CUSTOMER
(a) After the correction is done, the Results Slip or a new certificate with corrections will be sent to the principal or to the candidate (if not a student).
(b) A candidate who is found to be ineligible for a name change, will be notified to letter that the amendment was not made due to lack of qualification.
